All of our forms exist as online forms for your convenience. Simply click the online forms link below for the form that you require and complete that form onscreen. Remember to submit the form when you are finished. Note you only need to complete the online OR the manual form NOT both.
- Book Online (Use this form to book our services online)
- Online Song Database (Allows you to build a song request list for your event)
- Wedding Planning Form (Required by all clients who have booked us for their wedding)
- Master of Ceremonies (MC) Form (Only required if the DJ is providing MC services)
